To confirm enrolment we require a signed copy of your enrolment form to be received by Little Sports Star and payment to be made within 5 days of your booking otherwise your booking will lapse. We require all payments to be made prior to your child commencing the program. If the session you nominate is full, we will notify you as soon as possible.

Enrolment Conditions:

1. Your booking will lapse if we do not receive payment within 5 days of your booking.
2. Payment is required prior to your child participating in our program. Once the program commences, no refunds are given.
3. Your child's enrolment will be confirmed upon payment and receipt of this enrolment form, signed by you.
4. We reserve the right to terminate enrolment without refund, due to dangerous, disrespectful or disruptive behaviour.
5. By signing this form you consent to us photographing your child for use in advertising or marketing of Little Sports Star.
6. By proceeding with your child's enrolment, you agree to these conditions and to the Important Terms of Enrolment as follows.

Important Terms of Enrolment:

1. We reserve the right to cancel, alter or re-arrange individual sessions or an entire program in the event of venue problems, insufficient enrolment or other factors which are unforeseen or are beyond our control. If one session is cancelled, an alternative make-up session will be arranged.
2. You authorise Little Sports Star and our employees to provide and/or obtain medical assistance for your child as they see fit and you agree to meet any and all expenses that arise as a result.
3. You agree to advise on enrolment or immediately you become aware of any aspect of your child's physical or mental health or behaviour that may make them susceptible to injury or harm or that may place any other person involved in our program at risk.
4. You acknowledge by signing this form that sporting activities can be inherently dangerous, particularly when involving young children, that it is impossible to exercise complete control over young children and that unexpected actions by my child or by other children may cause or contribute to injury to them or to others, notwithstanding our endeavours to provide a safe program. You agree to accept the risk of injury, loss, damage and liability arising from your child's participation in the Little Sports Star program and you agree not hold Little Sports Star liable in any manner whatsoever should injury, harm or damage occur to you or to your child or to your property.
5. By signing this form, you hereby agree:
   1. to release and discharge Little Sports Star, its directors and employees from any claim, action or demand of any nature, resulting from any injury to me or damage to my property, that but for this clause may have arisen from or be connected with the involvement of your child in a Little Sports Star program or your own attendance at a Little Sports Star program;
   2. to indemnify and keep indemnified Little Sports Star, its directors and employees, against all losses, damages, penalties, suits, proceedings of any kind, costs, and expenses with respect to, or arising as a result of:-
      1. any injury to my child, to any of my other children or to myself, or damage to my property or to the property of my children, however caused; or
      2. ii. injury, loss or damage to any other persons or their property, including spectators, guests and other persons, arising from or connected with my conduct or the conduct of my child, however caused;
   3. that Little Sports Star, its directors and employees shall not be liable for any loss or damage occasioned to you, your child, or your property, including your heirs, executors and administrators in respect of any damage, injury, loss, or cost incurred or sustained by you, or your child however arising of whatever nature and without limiting the general nature of this paragraph, whether arising out of negligence or contract and whether such loss, damage, injury or costs has been caused or contributed to by any act, omission or negligence on the part of Little Sports Star or its directors or employees; and
   4. that this document may be pleaded as a bar to any action, suit or proceeding commenced now or taken at any time by you against Little Sports Star or it directors or employees with respect to any claim that you may bring for matters mentioned above.
6. A reference in this form to "Little Sports Star", "we" or "us" refers to CM Physio Pty Ltd ACN 130 244 147 trading as Little Sports Star, and where applicable includes reference to its servants, agents, contractors, employees and all other persons and companies associated with it or with, the owners of the land and buildings from which Little Sports Star operates its programs, and their respective servants, officials, representatives or agents. A reference to "you" refers to the parent/s or guardian/s named on this form or who submitted an enrolment booking or signed this form. A reference to "your child" includes your child named in this form and where relevant, to any of your other children present at the program venue.
